Just recently we have been using Jotform for creating our online forms. Jotform makes it so easy to create booking forms for our meals and trips. It enables us to take your bookings and collate all the information that you have supplied. Our event organisers are able to access the information they need in one place and it is really simple to use.
I would thoroughly recommend Jotform if you need to collect information or create surveys give it a try.
